Adjunct Faculty Change of Address or Name

Address Changes:

Address changes may be submitted by email (preferred) or in writing. If they are submitted by email, they must come from your Webster email account. Include your name, ID number, your new address and your phone number (whether or not it has changed). Send your email to Theresa Altmann at altmantm@webster.edu.

If you elect to submit your change of address in writing, include information as above. You must also include your signature. Send it to Theresa Altmann, Office of Academic Affairs, Webster University, 470 E. Lockwood, St. Louis, MO 63119.

Name Changes:

A name can only be changed by submitting a copy of the social security card showing the new name.  You may scan and submit a legible copy of the new social security card to Theresa Altmann at altmantm@webster.edu.   Or, the copy can be mailed to Theresa Altmann, Office of Academic Affairs, Webster University, 470 E. Lockwood, St. Louis, MO 63119.
