Data Request Form

Please complete the form below to request information from the Office of Institutional Effectiveness. In general, data will be provided only in the aggregate: all requests will adhere to guidelines set forth by the Family Educational Rights and Privacy Act (FERPA). Please give us as much notice as possible, as some requests take longer than others depending on the nature of the data requested.

Providing as much information as possible in your request about how you are going to use the data will allow the Office of Institutional Effectiveness to complete your project so that it meets your needs.
