Student Health Services Requirements for Residential Students
Student Health Packet
All new residential students are required to submit a Student Health Packet prior to moving into campus housing. The Student Health Packet contains detailed information regarding submission of medical and immunization information and mandatory health insurance requirements.

Failure to return the Student Health Packet with insurance information to the Student Health Services Department prior to moving into residential housing will result in a financial hold to be placed on your student account. This financial hold will jeopardize your housing assignment and the ability to register or make changes to your current or future class schedule.
