For all of you translators or health workers out there feeling difficulty when 
taking health histories, if you go to the web site www.MetDental.com  and click 
on the practice resources center you will find a standard free to use multi 
language health history form  including Creole, Spanish and French.  The same 
illness is on the same line whatever the translation so it comes in very handy.
